CL-2: No person other than an individual duly licensed to render the same professional services, within this state, as this corporation has been organized for, may own shares of this corporation.

 

CL-3: Transfer, pledge, hypothecation, sale, assignment, gift or other disposition of this stock is restricted by a buy-sell agreement, a copy of which is on file at the offices of the corporation.

 

CL-4: This corporation is authorized to issue different classes of stock, or different series within a class. A full statement of the designations, relative rights, preferences and limitations applicable to each class or to each series within a class and the authority of the board of directors to determine variations for future series will be furnished to any shareholder without charge upon request to the corporation.

TERMS: The terms for firms with open accounts is Net 30 Days. For all others with or without accounts,

FLORIDA SALES TAX: If applicable Florida Sales Tax will be added to orders.
